Job Summary:
We are seeking a dynamic and experienced Community Health Nurse to oversee our clinical operations and ensure high-quality service delivery.
The Community Health Nurse will provide clinical services and Support the clinical services offered through our maternal and child health program, as well as support the mobile clinics and follow up visits.

Key Responsibilities of the post
- Assess patients and establish health care needs; plan and implement nursing care interventions based on patients’ health needs
- Provide appropriate healthcare service, including immunization, ante-natal care and administration of medication.
- Evaluate healthcare outcomes on patients and prepare individualized reports
- Conduct assessment of school health needs; plan, implement interventions and prepare periodic reports
- Ensure a tidy and safe clinical environment
- Maintain record on patients/clients personal and health condition/care
- Provide appropriate referrals to patients and clients
- Provide health education and counselling on identified health and socioeconomic needs of patients
- Conduct follow ups
- Carry out health outreach activities
- Plan and implement health interventions and prepare periodic reports
- Administer emergency care in critical situations, including first aid and lifesaving interventions.
- Participate in health awareness campaigns and community education programs to promote wellness and prevention.
- Assist in the collection and analysis of health data to improve service delivery and address emerging health trends.
- Maintain compliance with ethical and legal standards including patient confidentiality
- Participate in regular training and professional development to stay updated on nursing practices and healthcare advancements.
- Contribute to the development of health policies and strategies within the organization.
- Any other duties as assigned by the Clinical Coordinator, Public Health Manager, Managing Director and or CEO.
Qualifications
- Diploma in Community Health Registered Nursing (KCHRN).
- Valid practice license from the relevant professional body.
- Minimum of 3+ years’ experience in clinical practice.
- Strong organizational and leadership skills.
- Excellent communication and interpersonal skills.
- Proficiency in medical data systems and Microsoft Office.
- Ability to work independently and as part of a team
- Passion for nonprofit work and making a positive impact in the community.
- Ability to ride a Motorcycle will be an added advantage.
