Link2Care is a social enterprise based in Nairobi, Kenya, committed to advancing maternal healthcare in Kenya. We provide affordable, accessible, and high-quality ultrasound and point-of-care testing (POCT) services to underserved communities, ensuring that no mother is left behind.

We currently collaborate with 30+ health facilities across Nairobi, reaching almost 1,000 expectant mothers in low-income communities with essential antenatal care services and health education. By combining innovation with compassion, we aim to close the maternal health gap and build healthier futures for mothers and their babies.

As our community facilitator, you’ll facilitate effective community engagement/outreach activities and ensure the delivery of health-related programs by acting as a bridge between communities and Health Reach Diagnostics limited. Advance health outcomes through outreach, health education, referrals, and community mobilization. You’ll be on the frontline of shaping how expectant mothers.

  • Point of contact to coordinate health-focused on outreach community activities.
  • Identify and assess community needs, particularly in health and access to services. Build a baseline for program planning and tracking.
  • Coordinate with Community Health Workers and health facility staff to strengthen community case management—including referrals, follow-ups, and adherence support (maternal health).
  • Conduct home visits, health education sessions, and mobilization campaigns (e.g. maternal health).
  • Train, mentor, and supervise community volunteers or health promoters. Build local capacity for sustained health interventions.
  • Facilitate referral processes to health reach diagnostics, ensuring seamless access to appropriate imaging services for instance ultrasound.
  • Gather, document, and report on community outreach, health indicators, and referral outcomes.

Mandatory:

  • Diploma or degree in Nursing, Public Health, Social Work, Community Health, or related field
  • Minimum 3 years’ practical experience in community & medical.
  • Good interpersonal and communication skills.
