Aga Khan University Hospital (AKUH) is hiring a Nurse Navigator, Home Care Health Services to coordinate home healthcare and ensure continuity of quality patient care. The role is based in Nairobi and experience in clinical case management is required.

Applications are invited from qualified candidates for the above position based in Nairobi Kenya. The successful candidate will coordinate and support patients throughout their healthcare journey, ensuring seamless transitions from hospital to home care while improving patient outcomes and overall care experience.

  • Lead patient onboarding into the Home Health Program, including patient identification, home suitability assessments, care planning, patient and family education, and coordination of safe transitions from hospital to home.
  • Provide clinical leadership and oversight for home health nursing services, ensuring delivery of safe, evidence-based, patient-centered care.
  • Conduct supervisory home visits, support staff development, monitor clinical competencies, and coordinate ongoing training and professional education initiatives.
  • Coordinate multidisciplinary care, facilitate communication between patients, families, caregivers, and healthcare providers, and ensure an exceptional patient experience throughout the continuum of care.
  • Manage staffing and service coverage to ensure timely, uninterrupted care delivery while maintaining workforce compliance, licensure, and mandatory certifications.
  • Develop and maintain referral relationships with hospitals, physicians, and other stakeholders to support program growth and patient enrolment targets.
  • Lead quality improvement, patient safety, risk management, and clinical audit activities, ensuring compliance with organizational and regulatory standards.
  • Monitor clinical outcomes, workforce performance, and operational metrics, and prepare reports to support continuous service improvement and program effectiveness.
  • Registered Nurse from a recognized school of nursing or University
  • Current licensure with the Nursing Council of Kenya
  • Certification in Basic Life Support (BLS)
  • Minimum of 5 years’ clinical nursing experience in an acute care hospital setting.
  • Experience in Home Health Services will be an added advantage.
  • Demonstrated leadership and care coordination skills
  • Strong critical thinking and analytical abilities.
  • Ability to work effectively within multidisciplinary teams.
  • Strong organizational and time-management skills.
  • Ability to prioritize multiple responsibilities in a dynamic environment.
  • Self-driven with the ability to work independently.
